Relevant Person (Data Owner) Application Form ("Application Form") below has been created for the purpose of making use of the rights set forth in Article 11 of the Personal Data Protection Law No. 6698 ("Law"). A summary schema for its use is included.

If you submit your application to our Company through one of the methods below, we will finalize your application free of charge as soon as possible and within thirty days at the latest. However, if the transaction requires an additional cost, we may charge you the fee in the tariff determined by the Personal Data Protection Board (“Board”). In this case, we will inform you of the fee that may be charged from you.

You can forward this completed form to our Company in writing or by using your registered electronic mail (KEP) address, secure electronic signature, mobile signature, notary channel or your e-mail address that you have previously informed us and registered in our system.
